You are applying for a mid-level position within one of the divisions of the National Center for Chronic Disease Prevention and Health Promotion. As part of the application process, applicants are required to submit a professional portfolio that includes evidence of the applicant’s ability to apply health behavior theory to the development of a health intervention that is comprised of multiple strategies. The portfolio should contain examples of tangible strategies (e.g., brochure, audio or video PSA, blog) that best illustrate the applicant’s work as a public health professional.

To make sure your portfolio contains what is needed for this job, you need to create a hypothetical health intervention that addresses a health issue. You should choose a health issue that is presented in the Healthy People 2020 topics and objectives. The intervention should be evidence-based, theory-driven, and address the health issue at each level of the Social Ecological Model. The goal of the intervention is to change a health behavior associated with the health issue.

Your Performance Task assessment will consist of a PowerPoint (PPT) presentation with “speaker notes” and two (2) tangible strategies. A list of acceptable tangible strategies can be found in the “Presentation Instructions” below. Specific instructions for constructing the tangible strategies are included in the “Tangible Strategies Instructions,” which come after the Presentation Instructions.

The “speaker notes” for each slide has a paragraph requirement, so be sure to follow the instructions in the speaker notes section of the Assessment PPT Template. It is not necessary to provide a recorded audio narration as part of this assessment.

Presentation Instructions

The PPT presentation should be 13-16 slides in length (which includes the title slide and the reference slide). Use the Assessment PPT Template that is provided.

To successfully complete this project, you need to do the following:

Slide 1

Create a title slide that reflects the content of the presentation.

Review Healthy People 2030 website and choose a health issue from the site. Then, identify the target population being affected by the health issue. If more than one population is affected, choose only one to focus on for this Assessment. On slides 2-3 describe the health issue that you selected from Healthy People 2030 using current statistics and describe a target population affected by a specific health issue using demographic data.

The following resources can be helpful in selecting and justifying a health issue and population.

Identify a behavior associated with the health issue. For example, smoking (the health behavior) is associated with cardiovascular disease (the health issue).

Use the “Guidelines for Choosing a Theory” located in Chapter 12 of the Hayden (2019) textbook to help you decide which theory would best fit the health issue and behavior that you’ve selected for this assessment. Be sure to explain how you used the guidelines to make your selection. Choose only one (1) theory or model to apply to the entire intervention. Choose from the following theories and models:

  • Health belief model
  • Theory of planned behavior
  • Transtheoretical model /stages of change
  • Social cognitive theory
  • Diffusion of innovation

Using the health issue and target population that you identified and the theory or model that you selected, provide an overview of your planned intervention as a whole and the best practices that will be used to create it.

Tangible Strategy Instructions

  • Choose two (2) different strategies from the five (5) strategies that you conceptualized and described on Slides 7-11(12/13) of your presentation. These will be the tangible strategies that you create and submit separately from the presentation.
  • Revisit the Learning Resources in “Module 2, Learning Activity 2.” Read and follow the recommendations and best practices for the specific strategies you’ve chosen.
  • Select one or more constructs from your chosen theory to apply to each strategy.
  • Use the guidelines in “Simply Put: A Guide for Creating Easy-to-Understand Materials” (in Module 2, Learning Activity 2) to create each strategy.
  • Use the SMOG formula (in Module 2, Learning Activity 2) to calculate the readability level of each tangible strategy. Revise as needed to meet the readability goals of your intended audience.Note: You will need to include the readability score before you made changes and then, if needed, the score after you made changes. If changes were made, you also need to explain what you did to meet the readability goals of your intended audience.
  • Submit your two (2) tangible strategies as either a Word document, PowerPoint, or PDF. For example, brochures can be made using Word and billboards created using PowerPoint.
